Children’s Library Card Application PreK – Grade 5

Please fill out the form below for your child to get a library card. A library staff member will contact you within 2 business days to finalize your account and give you a library card number. This number will be good for a month. When you visit the library, bring your ID or a piece of mail with your name and Stratford address to pick up your child's actual library card, which will be good for three years.

In the event my card is lost or stolen, I understand that I am responsible for charges until the date I notify the library of the loss or theft of my card.
